BID DOCUMENTS OF ANNUAL RATE EMPANELMENT FOR BUILDING ACTIVITY IN WARD NO- 1 TO 13 IN PORBANDAR MUNICIPAL CORPORATION

Scope of work

Establishment of an annual rate contract for building construction activities including maintenance, repair, and new construction works across Ward No. 1 to 13 of Porbandar Municipal Corporation. The empanelment is for a 12-month period. Bidders approved will be available for undertaking construction assignments as per the Corporation's needs. This is a rate-based empanelment where contractors submit annual rates for specified building works categories.

Eligibility criteria

• B-class Civil Contractor and Above as per registration classification
• Must not be associated with consultant or entity that prepared design/specifications for this project
• Must provide declaration of non-association with design/specification preparation entities
• Must comply with Governing Conditions of Tender (GCT) in entirety
• All directors/partners/KMPs must not be blacklisted or debarred in preceding 5 years
• Must disclose all material adverse facts including litigation, insolvency, criminal proceedings

• Affidavit B: Non-Blacklisting/Debarment Affidavit (5-year history, insolvency status)
• Affidavit C: Material Information Disclosure Affidavit (adverse facts for 5 years)
• Form 1: Tender Fee and EMD Details (instrument particulars, UTR)
• Form 2: Bidder Particulars/Constitution/Authorization (legal status, PAN, GSTIN, signatory details)
• Form 3: Material Information Disclosure Schedule (9-category structured disclosure)
• Form 4: Litigation History (legal, insolvency, arbitration proceedings for 5 years)
• Form 5: Debarment/Blacklisting/Non-Performance Disclosure (5-year history)
• Form 7: Contract/Experience/Completion Certificate Details (past performance records)
• Form 8: Bid Capacity/Existing Commitments (ongoing works, contract values, capacity computation)
• Form 9: Key Plant/Machinery Availability (equipment ownership status, make, capacity, age)
• Form 10: Key Personnel Availability (qualifications, experience, deployment dates)
• Form 11: Financial Reports and Credit Facility (audited financials, banker details, credit access)
• Form 13: Conflict of Interest/Collusion/Related Party Disclosure (absence of conflict declaration)
• All documents must be duly signed, scanned, and uploaded online within deadline; EMD/Tender Fee in original via RPAD within 7 days of bid deadline
